Here is a checklist that you can give to your hosting provider:
▪Check they have .NET Framework 4.0 and SQL Server 2012 R2 or higher
▪FTP username and password
▪FTP server address
▪FTP directory
▪SQL Server address
▪SQL Server username and passwords
▪SQL Server database name
▪The ASP.NET user account needs write/delete access to the full website directory.
